Please review the information you entered is correct. If you find any information is incorrect, you can press "Modify" to amend.

You need to sign the application with your digital signiture:-

1. Input the PIN of certificate to confirm correctness of the input information and consent the declaration.

After completion of this screen:

1. Press "Sign and Continue" button to go to next screen. or
2. Press "Save" button to save this page in a file. or
3. Press "Print" button to print this page to a printer. or
4. Press "Clear" button to clear and re-enter the data.
